چکیده

Background and Aim: Anemia is a major clinical multifactorial problem in hemodialysis (HD) patients. Iron metabolism is frequently impaired and may complicate the management of anemia. Hepcidin is synthesized in the liver. It is thought to be a key regulator of iron homeostasis. Therefore, we thought to assess prohepcidin level and iron indices in patients on regular HD with and without associated viral hepatitis. Patients and Methods: Sixty patients (38 males and 22 females) with chronic renal failure (CRF) undergoing regular HD and twenty age and sex matched healthy controls were included. Peripheral hemogram, liver function tests, kidney function tests, lipogram, C-reactive protein, iron indices, serum prohepcidin, HBsAg, HBcIgM and anti-HCV antibody were tested for both patients and controls. Results: 22 Patients were positive for anti-HCV, 16 were HBsAg positive and 22 were negative for both markers. Mean serum prohepcidin and ferritin levels in HD patients were significantly higher than controls. There was significant increase in the mean serum prohepcidin level in HD patients with hepatitis B compared to those with hepatitis C. Prohepcidin had significant negative correlations with both hemoglobin level and total iron binding capacity in HD patients with hepatitis C. However, in multiple regression analysis, serum prohepcidin level was independently related to hepatitis, and triglycerides level in the whole group of HD patients. Conclusion: CRF patients on HD have elevated serum prohepcidin and hepatitis C patients have higher levels than those with hepatitis B. Prohepcidin was independently related to the presence of viral hepatitis and triglycerides suggesting the important role of the liver in hepcidin synthesis.

ارتباط پروهپسیدین با شاخص‌های سرمی در بیماران همودیالیز مزمن: بررسی 54 بیمار

Background: Prohepcidin, a liver-derived peptide with antimicrobial properties, is regulated by factors such as iron load and inflammation. Hepcidin is a central player in iron homeostasis. It downregulates the iron exporter ferroportin, thereby inhibiting iron absorption, release and recycling. Thus, prohepcidin increases the possibility of iron-limited erythropoiesis and development of anemia...

Serum prohepcidin levels in chronic hepatitis C, alcoholic liver disease, and nonalcoholic fatty liver disease

BACKGROUND/AIMS Patients with various chronic liver diseases frequently have increased body iron stores. Prohepcidin is an easily measurable precursor of hepcidin, which is a key regulator of iron homeostasis. This study investigated the serum prohepcidin levels in patients with various chronic liver diseases with various etiologies. METHODS Serum prohepcidin levels were measured in patients ...

بررسی وضعیت ذخایر آهن و شیوع کم خونی فقر آهن در مبتلایان به بیماری مزمن کلیوی در مراحل پیش از دیالیز (pre-dialysis) و همودیالیز نگهدارنده

Background and Objective: Anemia is one of the major disabling consequences in chronic kidney (CKD) disease which has different etiologies .Inadequate production of Erythropoietin (EPO) is the main cause of anemia. Iron shortage is the other important cause. The aim of this study is to evaluate Anemia Prevalence and iron storage in CKD patients in pre-dialysis and hemodialysis stages.   Mater...

Lower serum prohepcidin levels associated with lower iron and erythropoietin requirements in hemodialysis patients with chronic hepatitis C

BACKGROUND Patients with chronic HCV infection have increased liver iron. Recently identified protein hepcidin synthesized in the liver, is thought to be a key regulator for iron homeostasis and is induced by infection and inflammation. Lower erythropoietin and iron supplementation requirements were previously reported in HD patients with HCV infection. We investigated the association of prohep...
